Although understanding of the cellular and molecular mechanismsof atherogenesis has greatly accelerated over the last decade, surprising modern concepts of its basic pathophysiology have existed for over a century. In the mid-nineteenth century, Virchow first postulated that atherosclerosis was a chronic arterial disease secondary to vascular injury (Virchow 1856). The belief that atherosclerosis involved thrombosis and endothelial inflammation was raised several decades ago by Daguid (Daguid 1946) and French (French 1966) respectively. Despite a clinical perception for many years that atherosclerosis is a degenerative disease of aging, it is now clear that the most significant basic feature of the disease is the presence of a chronic but active inflammation of the arterial wall which, over many years, results in intimal fibroproliferation and vascular occlusion.
