In recent years, the concept that atherosclerosis has many pathophysiologic characteristics of a chronic inflammatory process has lead to a reappraisal of the mechanisms of the acute coronary ischemic syndromes. A particularly distinctive feature of the natural history of coronary artery disease well known to physicians is the sporadic nature of its clinical presentations. Thus in most patients, the disease characteristically runs a chronic course of relatively stable angina, to be interrupted acutely by unstable angina, myocardial infarction or sudden death. While all these syndromes are due to a deficiency of myocardial blood supply relative to demand, the biologic basis for this pathophysiologic abnormality with regard to cellular and molecular mechanisms is less clear, but has been the focus of much study. Many lines of evidence now suggest that these cyclical episodes of acute coronary ischemia are due to activation of inflammatory mechanisms in atherosclerotic lesions. In addition, these have many features that strongly suggest that immune processes are involved either in the initiation or the amplification of the inflammation.
