Insulin-like growth factors (IGFs) and their receptors exert their effects on a variety of sarcomas through autocrine, paracrine and endocrine mechanisms. In addition to regulating the proliferation of normal as well as malignant cells of mesenchymal origin, IGFs and their receptors play a central role in transformation, survival, metastasis and differentiation of sarcomas. Most of these effects are mediated through the IGFI receptor (IGFIR). Three pediatric sarcomas, namely rhabdomyosarcoma, osteosarcoma and Ewing’s family of tumors illustrate the pleiotropic effects of IGFs. In this chapter, we review our current understanding of the role of IGFs in the pathogenesis of these tumors. Mechanisms of overexpression of IGFII and the IGFIR receptor in these tumors are discussed including epigenetic modifications and effects of mutant p53 on the promoters of these genes. The potential of the IGF signaling pathway as a therapeutic target of sarcomas will also be discussed.
